Neuroaffirming Autism and ADHD Assessments

At Stardust Neuroaffirming Wellness, we understand firsthand the complex journey of finding and understanding our unique neurotype as a neurodivergent human. Our approach was created by queer, PDA, autistic, and ADHD folx who understand the lived experience of being a neurodivergent human in a neurotypical world. In addition, our assessments are done by a Vanderbilt-trained, holistic, Autistic and ADHD, holistic psychiatric mental health nurse practitioner.

Assessment Process

  1. Schedule your evaluation in our scheduling portal.

  2. You will receive a confirmation email with instructions to get set-up in our portal.

  3. Pre-interview Neurodivergent paperwork, gathering of social, medical, mental health, and developmental history, & pre-interview self-assessments

    You can expect to spend 1-2 hours completing questionnaires, history, and self-assessment tools before the assessment appointment.

  4. Collaborative interview
    We’ll have a conversation about:

    • Your life experiences and history

    • Interests and passions

    • Communication styles

    • Sensory experiences

    • Relationship patterns

    Bring anything you need to feel comfortable:

    • Support person(s) or items

    • Snacks or drinks

    • Fidgets or comfort items

    • Crocheting, knitting, etc supplies 

    • Items that help me understand you better (photos, letters, collections, literally anything you would like)

    • If you prefer, you don’t need to bring anything at all! 

    Please dress comfortably—I will be too :)

    Autism Evaluation: 1.5-2.5 Hours (if more time is needed, we will schedule an additional interview appointment)

    This process utilizes the MIGDAS-2 and a structured-interview rooted in the The Adult Autism Assessment Handbook which was created by autistic psychologists to center the experience of neurodivergent humans. In addition quantitative measures such as the SRS-2 are used.

    ADHD Evaluations: 1-2 Hours

    This process utilizes the Diva-5 in addition to quantitative tools like the CAARS-2.

  5. Optional Collateral Information & Formal Screeners

    The evaluator and client may wish to gather additional information from family members, partners, educators, service providers or other supportive folks, if appropriate, and only as agreed upon with consent of the person being evaluated. In addition further formal quantitative screeners will be sent. The provider may also send you other measures discussed at the appointment to further investigate possible overlapping conditions if needed.

  6. Evaluation Report

    I will pull together all information gathered and write a short report which may include evaluation outcomes, diagnosis if appropriate, as well as recommendations. We do also offer full comprehensive 15-20 page reports with sensory profiles (unfortunately insurance does not cover these) for an additional fee if interested.

    Please note that we do not use or offer evaluations using the ADOS-2, if you are seeking services or accommodations, please ensure that they do not require the ADOS. We have chosen not to offer the ADOS for a variety of reasons including it is not neruoaffirming and does not center the experience of autistic people.

  7. Follow-up Session

    This is typically scheduled approximately 4 weeks after all information has been gathered from client and collaborating sources. This session is 25 minutes where we will review outcomes of evaluation, diagnostic report, recommendations, and any questions you may have.

Costs

We are currently in-network with most Blue Cross Blue Shield plans, please check your individual insurance plan.

For out-of-network or cash pay clients, or those who prefer to not use insurance, we offer a superbill for OON reimbursement if eligible, as well as free 6 month payment plans. You can reach out here for more information on cash rates for evaluations.

In order to make evaluations more accessible, we also offer an equitable sliding scale as resources are available. For more information on sliding scale rates and availability, please contact us.

Please note, must be physically located in Iowa, Washington, or New Mexico at time of assessment/appointment (some clients chose to travel for assessment interview).
